Abstract

In this paper, we first review the central role traditionally played by community in structuring professional work, especially the case of medicine; we then document the growing influence of market and hierarchy principles in the progressive restructuring of professional work over recent decades; and finally, we discuss current new combinations of market, hierarchy, and community principles.
