Conservatorship Financial Care Plan Template

Abstract

The following is a care plan template derived from the “Conservatorship Care Plan” Form of the Superior Court of California for the County of Los Angeles (PRO-023, revised July, 2008). The entire form is only four pages and includes both the personal and financial needs of the conservatee. This template expands upon the form and includes a deep, thorough analysis of the conservatee’s financial assets, liabilities, needs, desires, and goals. Each section will reference the form and will include concerns, recommendations, and rationales from the conservator’s perspective. The concluding section contains an action item list detailing all steps that must be taken in the near future to implement the conservator’s recommendations, the party in charge of each step, and by when each step will be completed.
