Epidemiological and Transmission Patterns of Pregnant Women with 2019 Coronavirus Disease in China

Abstract

Background: The current outbreak of coronavirus disease (COVID-19) is a public health emergency of international concern. We report the epidemiological and transmission patterns of 103 pregnant women diagnosed with COVID-19 nationwide in China.

Methods: We included a total of 103 pregnant women with COVID-19 reported to the Chinese Center for Disease Control and Prevention (China CDC) from January 16 to February 8, 2020. Demographic characteristics and clinical features were described. The epidemic curves were constructed by key dates of disease onset, and case diagnosis, while onset-to-diagnosis curves were constructed by fitting a log-normal distribution to data on both onset and diagnosis dates. Furthermore, multinomial logistic regression models were performed to assess the risk factors of severe/critical disease.

Findings: By February 8, 2020, 103 pregnant women with COVID-19 had been reported to China CDC, including 81 (78·54%) laboratory-confirmed cases, and 22 (21·36%) suspected cases. The median age of all patients was 31 years (Interquartile range: 27-38). For the severity of patients (including both confirmed and suspected cases), 42 (40·78%), 47 (45·63%), 12 (11·65%) and 2 (1·94%) patients were diagnosed as mild, common, severe or critical cases, respectively. There was no significant difference across different severities of illness by age group. There was a rapid increase in the number of pregnant women with COVID-19 at the early stage of the epidemic, and then a gradual decrease in general. Over a half (53·40%) of pregnant women were infected in Hubei province, while there were less than 5 cases in most other provinces. The multinomial logistic regression analyses show that the longer the time from onset to diagnosis, the more severe the illness.

Interpretation: As the first national study on epidemiological and transmission characteristics of pregnant women with COVID-19, it provides preliminary information about the scope and severity of coronavirus infection during pregnancy in the current epidemic.
