Abstract

Purpose: To examine the possible relationship between miosis and improvement in near vision for reading after special topical formulation (FOV drops). This study was developed in a private practice center (Vejarano Foundation, Popayan, Colombia).

Methods: This was a prospective interventional study of consecutive presbiopic subjects who were tested for vision and pupil diameter before and after two hours of instillation of a special pilocarpine drop formulation for presbyopia. The drop contains both pilocarpine and phenylephrine. Included subjects were all consecutive presbiopic patients with distance refractive error between -1.75 and +2.00 D aged 40 to 70 years who accepted trying drops for reading purposes instead of glasses. Pre- and Post- drop instillation patients underwent subjective refraction, photopic and scotopic pupil diameter, and distance and near, monocular and binocular distance and near unaided logmar visual acuity testing. These main outcome measurements were correlated and studied with Student t tests.

Results: The study includes 446 subjects with mean age 50.5 +/- 5.8 years (227 were females, 50.9%). The mean spherical equivalent was +0.099+/-0.495 diopters and did not change significantly after the FOV drop. The scotopic pupil diameter decreased after the drop by 0.94+/-0.98mm. The near logmar visual acuity improved two lines after the drop. There were no significant differences in near visual acuity gain between those with smaller or bigger pupils (p=0.18).

Conclusions: The improvement in near vision with pilocarpine + phenylephrine FOV drops is independent of pupil change.
