Who Killed Managed Care? A Policy Whodunit

Abstract

Several observers have declared that managed care is dead. If so, who or what killed it? Any number of suspects might be implicated, including physicians, patients, the media, the courts, legislators, hospitals, employers, and so on. After examining the suspects' motives and opportunities to kill managed care, I conclude that the wounds were self-inflicted. Despite having a good concept and product to sell, the managed care industry self-destructed because of a poorly implemented strategy. To be sure, implacable opposition and political interference made survival a difficult challenge. These same collective forces will confront whatever system replaces managed care.
